       Reporting Discrepancies and Verifications Using
       "Aid to Navigation Report" CG-5474 (ANSC-7054)

  1. Other than the preprinted Aid Verifier forms, the only form currently approved for reporting aids to navigation (AtoN) discrepancies and verifications is the "Aid to Navigation Report" form CG-5474(AUX)(Rev. 5-95). Your materials staff officer can order these forms from the ANSC using ANSC-7054 or you can access the form online.

  3. AtoN discrepancies should be reported, one per form CG-5474, by all Auxiliarists. Check the DISCREPANCY box in the upper right-hand corner of the form. Fill out the form completely and use the COMMENTS section to better describe the discrepancy. Important: Any discrepancy involving lights, including private aid and bridge navigation lights, should be reported immediately via VHF marine radio or telephone to the appropriate Sector. This contact should be recorded in Section II of the CG-5474 form.

  5. For bridge discrepancies, print the word "BRIDGE" next to the verification / discrepancy check boxes at the top of the form.

  7. Qualified Aid Verifiers may also use the CG-5474 form, in lieu of using the preprinted forms, to report verifications that are "watching properly". Check the VERIFICATION box in the upper right-hand corner of the form and leave the top half of Section III blank. In the COMMENTS section, enter "The following aids are watching properly:" and then list the aids (Light List Number, River Mile, Aid Name and time of observation for that Aid).  Do not mix Bridge verification reports and PATON verification reports on the same CG-5474 form.
     

  8. For aids to navigation verifications and discrepancies, distribute the copies of the CG-5474 as follows:

    • Original (Copy 1) and Copy 2 - For all aids except bridges, submit to the Coast Guard unit having responsibility for the area. Bridge reports should be sent to: Roger Wiebusch, Bridge Administrator, Western Rivers Operations(ob), 1222 Spruce Street (Room 2.107F), Saint Louis, MO 63103-2832. It is very important that aid discrepancy reports are accurate and are delivered immediately to the appropriate Coast Guard unit (cutter) or to the bridge administrator (for bridges).

    • Copy 3 - Send to ADSO-AN, who will process it further to DSO-AN..

    • Copy 4 - Distribute in accordance with your Division's established policy.  (Typically, this is sent to the SO-AN with a copy being sent to the FSO-AN.)  IMPORTANT: If the Bridge or PATON that you are verifying is located outside of your Division's assigned AOR, then send a copy to the Division SO-AN in whose AOR the Bridge or PATON is located.

    • Copy 5 - Is retained by the observer.

  10. For aids to navigation chart updates, distribute the copies of the CG-5474 as follows:

    • Original (Copy 1) and Copy 2 - Submit to the Federal (e.g., the Corps of Engineers) or State Agency having charting responsibility for the area. Do not submit form to NOAA unless the update is for a NOAA chart. (There are no known NOAA charts for any of lakes and waterways in the Eighth Western Rivers Region.)

    • Copy 3 - Send to ADSO-AN, who will process it further to DSO-AN.

    • Copy 4 - Distribute in accordance with your Division's established policy.  (Typically, this is sent to the SO-AN with a copy being sent to the FSO-AN.)  IMPORTANT: If the Bridge or PATON that you are verifying is located outside of your Division's assigned AOR, then send a copy to the Division SO-AN in whose AOR the Bridge or PATON is located.

    • Copy 5 - Is retained by the observer.

Reports of "No Activity" are not required.

AUXDATA information will be taken from the Activity Report - Mission form (ANSC #7030)
